Nominate an inspirational individual to carry the Olympic Torch in the Paralympic Torch relay!
Is there anyone in your life who has truly made a difference to the lives of others, who deserves to be nationally recognised? If so, why not nominate them to carry the Paralympic flame in 2012.
To apply, you need to write a short description of who you are nominating and why. You should include how the person has been a source of inspiration and encouragement and what kinds of determination, courage, passion and equality they have showed in helping others in their community to succeed.
By nominating that special someone, you will not only be giving them an opportunity of a lifetime, but you will also being doing your bit to help support and promote Vitalise!
To nominate, you can either pick up an application form in your local Sainsbury’s or visit their website to complete it online and see the full terms and conditions.
Nominations close on February 14th 2012, so start applying and remember to keep us informed of your progress!
